Eligible MedStar Family Choice members can participate in one of three ways: To be eligible for the Diabetes Prevention Program, you must meet all of the following requirements: You have Maryland HealthChoice (Maryland’s Medicaid Managed Care Program). You can use this service anytime, day or night, all year round, from your own home. Doctors from MedStar Health will check you for common sicknesses and injuries and help you with treatment plans. MedStar Family Choice can assist your office with utilization management issues (such as obtaining pre-authorization), care coordination, and case/disease management services. Beginning on May 1, 2024, MedStar Family Choice is required by the Maryland Department of Health to have a copay (cost) for most prescription medicines. For most formulary (covered) brand name or generic medicines there is a copay (cost) of $1.00Someone will reach out to complete the enrollment process.
