My LC9s came back with a new guide rod that was different from the original. Note the forcing cone behind the guide rod head in the pic below. This cone centers the guide rod head in the slide hole during the last bit of forward slide travel to prevent a possible hang-up due to misalignment. Another thought--I think you can unload the pistol, hold the slide completely to the rear while inserting and withdrawing an empty mag. and tell whether or not the side stop spring is functioning. It should drop the slide stop when the mag. is lowered. Sounds like your grip is pushing up on the slide lock.

It may be that the grip frame had some malformation that that kept the slide lock from engaging. Bought the ruger lc9s fired 250 rounds threw it and the slide kept locking open and double staking. Called ruger sent the gun back at no cost to me. They repaired it in 3 days and sent it ba k 2 day so I had it back with in 7 days. Sofar works perfectly. Better than before I would say. Customer service was great had a pleasent experience.
