Pausing it for 10s, 30s, 60s or more doesn't help. That makes me believe that Plex could do better in terms of buffering. Is there any configuration I can do to improve this buffering? I am ok in watching the stream with a considerable delay, as long as it is smooth. I tried to set "Transcoder default throttle buffer" to 180s, but it doesn't help.

Well im hoping it will fix my buffer problem. I have a problem where my videos buffer frequently, but everything shows that it should be running fine. My cpu less than 20% usage, gpu under 10%, and my upload speed is above 50mbps. Still a TV show will buffer every minute or so when being transcoded.
